But did you know your Blue Cross of Idaho benefits includes support from care managers who can help you through healthcare challenges? If you are currently or expect to be dealing with multiple or complex health problems, Care Management can help.

What is Care Management?

With the help of your own dedicated care manager, you’ll better understand your conditions, learn how to take an active role in your health and get help with navigating any obstacles you run into.

This program includes healthcare planning, care coordination among providers, advocacy and more.

Care Management offers extra help to members dealing with multiple or complex health conditions like serious trauma, transplants, spinal injuries, cancer, behavioral health or multiple chronic illnesses.
